If you are an Asthmatx employee or partner, and you would like to obtain files such as presentation slides, videos or practice enhancement collateral (“Materials”) through the Asthmatx Extranet http://www.btforasthma.com/extranet we will ask you for your email address and provide you with a temporary password to a password of your choosing. The website may also contain other interactive areas or forms wherein patients, physicians, and others may register or provide their personal information.
By registering or providing personal information or information of any kind, users of this website represent that the information provided is true, accurate, current, and complete. The sender of any communication or information to this website or to Asthmatx, Inc., shall bear all responsibility for the content, legality, truthfulness, and accuracy of such communication or information.
